Youth Programs Family Care Coordinator

Reports To: Senior Advisor, Youth Programs

 

DESCRIPTION

This position coordinates Youth Programs intakes and referrals; oversees communication with Youth Programs families, identifying special needs or concerns for bereaved children and teenagers participating in TAPS programs; and acts as a liaison for casework, survivor services, and the helpline to ensure a smooth transition of care. 

RESPONSIBILITIES

Intakes and Referrals

  • Create and oversee an intake process for orienting new families to Youth Programs
  • Oversees all intakes and referrals made to Youth Programs from within and outside of TAPS
  • Collaborates with case management to ensure special needs identified in children, teens, or families receive the proper referrals

Family Engagement

  • Create a plan, oversee, and implement ongoing contact and communication with Youth Programs children, teenagers, and families
  • Act as a liaison between casework, survivor services, and Youth Programs
  • Receive helpline calls regarding Youth Programs address special concerns for families with children
  • Serves as the point of contact for fielding calls and requests regarding bereaved children, teenagers, and families
  • Collaborates with other Youth Programs staff for program planning to ensure meaningful, supportive experiences for children, teenagers, and families participating in Youth Programs

Youth Programs Advisory Committee

  • Serves as a member of the Youth Programs Advisory Committee

Publications and Activities Database

  • Collaborate with the Senior Advisor of Youth Programs to provide content for Youth Programs publications and activities database
  • Collaborate with the Senior Advisor of Youth Programs to ensure that activities in the database align with established standards and criteria

 

QUALIFICATIONS

  • Four-year degree in social work, counseling, or equivalent, master’s degree a plus
  • Proficient in Microsoft products
  • Excellent interpersonal skills
  • 2-5 years’ experience in program development and management
  • Must be creative, accurate, and detailed-oriented
  • Needs to possess strong organizational and time management skills and the ability to manage competing priorities
  • Flexible and able to adapt to special needs
  • Excellent oral and written communication skills
  • Ability to travel
